Sounds like classic low self-esteem issues. Shes convinced that everyone around her really doesn't like her and is constantly on the lookout for evidence to prove her theory. Trouble is, if Hisao and Lilly are too careful around her and try too hard to make her feel worthy, she goes into "You guys treat me like a baby" mode. Take about having to walk a narrow rope.
I like to think that with Hisao in the picture, having little birthday parties and gifts might become a regular thing between the three of them and maybe, she'll eventually realize that its something that everybody deserves.

She blames herself for her parent's death. She thinks if she wasn't born, her parents wouldn't have died trying to save her. This whole thing makes her a really difficult character to work with. If someones down, most of the time you make a big deal over them to cheer them up and convince them that they're worthy. In Hanako's case, it just makes her feel worse. How the heck do you deal with that?
I'm wondering if they might have better luck with shared events instead. Ignoring her birthday, and instead having little parties and presents to mark the anniversary of she and Lilly's first meeting, or the day she and Hisao became a couple. At least then the spotlight wouldn't all be on her.

That's actually a pretty good idea.
