I just realized how hard I'd be laughing if there was a scene where Hisao pops a massive boner for no reason at all... although we could say that's it's a side effect of some new medications, since that would give reason for it to be extra-large. and to keep with the topic (boners), what I want to happen is a really awkward scene with Nurse where Hisao has to say that he wants to have sex with his girlfriend but can't because of his meds, so could Nurse please change them. that would be hilarious.

I'm interested in seeing more mental conditions explored. Not the sort that would generally land you in an institution, but which make aspects of modern life exceptionally difficult or different for the affected than most others. Histrionic disorders, anhedonia, Asperger's, OCD, et cetera.
