It was the shock from the heart attack and just the general tense situation from the confession that got his heart rate going and triggered the attack. So he would have to pace his relationship to the point where the sex would be natural, not a sudden occurrence. I'd assume eventually he would tell who ever he was with about his condition, seeing as he looks fine in comparison with some of the people there, they would be curious. So they would obviously know that if they push too hard they could potentially kill him. I doubt he'd have that problem with Hanako, she'd probably have the heart attack.

Hisao will die at the end of act 2. The rest of the game will be spent as Kenji fighting the evil feminists.

Actually depending on your choice of girl in act one and two that girl will be the leader of the evil feminists with the game ending in a duel between that girl and Kenji as the feminist empire headquarters crumbles around them.

Apparently people with most people with heart conditions can usually have normal sex lives too. Although it does depend on severity; if there are any complications, etc. And I read that exercise will help build stamina, too.

About Hisao "suddenly" collapsing due to Iwanako's confession: You have to remember that Hisao's condition was probably getting worse without him knowing all the time. It's just unfortunate (and strangely appropriate) that his heart gave out at that moment. Realistically, he was a ticking timebomb, and if he didn't have a heart attack during the confession, it was bound to happen eventually, and who knows what would have happened then? As for why it didn't happen before, you'd just have to call that luck, combined with a semi-inactive lifestyle.

Off topic, but the idea of Hisao having his heart attack later seems interesting. If that were the case, he'd be with Iwanako for awhile. Forgive me if this idea has already been explored, I haven't read the fanfiction forum in a long time.

Sort of on topic there _is_ one thing that I've always wondered about:

The game mentions Hisao playing soccer, although rather unenthusiasticly. He's probably been stressed at least once in his life, probably tried to catch a bus like his life depended on it once or twice. You know, 'cause he was going to be late for that new eroge's release.
So how come the thing that finally caused the heart attack was a confession? Seriously, a confession! He's a high schooler. He's fapped for years, probably. He's watched romantic movies. He's been infatuated with Iwanako for quite a while and obviously has tried to get close to her, at least physically, for a while. (You know, to eavesdrop.)
His life has been full of situations that rise one's blood pressure and make your heart beat like crazy. Why only once he got a confession? Why?!
The propability is so absurd it baffles me day by day, week by week. I cannot sleep, I cannot love, I no longer even have the beat. So why?

Yeah, that got a bit out of hand. Anyhow, if I had to point out one flaw KS has in it's story it would be that one. Why's the heart attack so late in the game?

Doubtful. I expect the only ones who would stay with him with a non-functional penis would be Shizune, Hanako and Rin.
