In Akiha's "Good" End she is reduced to a blood-sucking vegetable, while in her True End, she survives, but the Main Character dies.

Actually, the first one's her Normal End; Akiha doesn't get a Good End. Also, it's implied (and confirmed in Kagetsu Tohya) that Shiki survived the True End...somehow.

Hmm. I didn't read that many VN's yet... So far the only one with the True End/Good End formula was Tsukihime.
Is that something that happens often?

Not really.

With the little sister Akiha you do not get a good end...hinting at that incest relationship(even if not blood related) is bad. In my opinion that was the only thing I did not like about Tsukihime.

You don't get a really good end with Akiha, because in one end you die and in the other she turns into a vegetable. (Yes, I know they did a backflip on the first in a sequel.
I don't think relationships between siblings are evil per se. It's not for me, but people should be true to their feelings. True love can never be a bad thing.
Doesn't apply to Tsukihime anyway, because as you said they are not related.
