What third ending should Lilly and Shizune had?
The others have good, bad, and neutral endings, but Lilly and Shizune only have the good or bad (or good and neutral depending on how you look at it.) So, if it had been up to you, what THIRD ending would you have liked to see for those two routes?
... Granted, I guess that means Shizune would need another question somewhere in her route. Either that or some of the things you decided in Act 1 would directly affect her ending.

Shizune should have had an ending where you convince her to hook up with Misha. There may or may not be a scene where she and Misha fuck, and you watch through an opening.
Lilly should have had an ending where the plane back to Scotland crashes somewhere near Yamaku, and you go to investigate it, finding a mangled, mutilated body amongst the wreckage. It's barely recognizable as human, given the contortions of the limbs, the way it's impaled by large and small pieces of metal, and the blood that seeps over nearly every inch of it. Then you notice the hair, long, matted by blood, and distinctly blonde... No, there's no way this is Lilly! She could still be alive, just trapped underneath something, or she could have already escaped. You summon up the courage to take a closer look at the atrocity, hoping to extinguish the part of you that says that Lilly is dead. Whatever it takes; anything to know if she might have survived! It... appears to be holding something in its hands. A small wooden music box. You warily retrieve it and wind it up, then let it run.

Lilly only has one ending as far as I'm concerned, since there's no additional content for letting her go without engaging in ridiculous theatrics to stop her.
G - Hisao actually puts up a fight about her leaving and secrets, successfully convincing her to tell her parents to stuff it.
N - As above, but he fails to convince and he storms off. Add an extra scene or two of Lilly working to apologize before she leaves so they don't end on poor terms.
B - Passive acceptance leads to the current ending, but Lilly does not show up at the hospital. Due to a second long absence he's going to have to repeat the school year.
As for Shizune, I think I'd like to see her current good/neutral ending made more explicitly an amicable parting, then add a clearer good ending where she and Hisao are planning on going to the same college. As for accessing it... perhaps add a choice involving the trip Jiggy wants her along on, encouraging her to go or to stay with you? Needs alternate scenes of Hisao and Misha handing SC business without her and Shizune returning from the trip with a better relationship with her family and Jiggs actually more approving of the glue-huffer.

Lilly bad end should be of her walking in on Hisao and Hanako fucking each other brains out. After seeing them go at it, Lilly writes a note declaring her hatred for both of them and commits suicide, blaming Hanako and Hisao for her suicide.
Shizune's neutral ending is they simply break up but Hisao never cheated on her with Misha and they all remind life-long friends. Shizune becomes a big sister to Hisao.

To me the Shizune endings were Good and Neutral.
Sadly, I think the 'bad' ending would have involved Shizune and Hisao failing to help Misha overcome her depression -or see past it, as it were- and ended with a funeral where they end up breaking up over Misha's suicide. That wouldn't be in the spirit of the game even if it could have been a reality, which could be why no such possibility exists in the game.
The Lilly 'Bad' ending is more difficult to quantify. I don't think it could be as simple as Hisao actually dying as a result of his attempt to chase her down. I also don't think it would be as absurd as a plane crash - which would be tragic, of course, but wouldn't follow the theme of the story. Though, perhaps after the airport incident it could be possible for the situation to tear them apart as easily as it brought them together. In that case, the 'bad' ending could have been even more heart-wrenching where Lilly leaves for Iverness without so much as a regret.
I think in both cases, it didn't really need to be said or it would have been redundant. Sometimes a story is made better by holding back some of the details.

Anyway, another possible bad end for Lilly's Route:
Hisao is lying on his bed trying to go to sleep. He had a bit of a long day with Lilly and just want some rest for the next day. The one second where he thought he could get some sleep he here's a knock on the door. Annoyed, he answered the door and to his surprise, he sees Hanako in her casual clothes. During some small talk, Hisao notices that Hanako has alcohol on her breath and her speech is a bit slurred.
Noticing that she has been drinking, he tells him that she needs to leave his room and go back to her own dorm quickly before anyone notices her drunk. Hanako refuses and kisses Hisao on the lips, while knocking the surprised lad on the ground, placing both of them in an awakrd pose with the door wide open. As Hisao tries to her off of him, Hanako presses down on his shoulder and tells him to listen to her.
CHOICES:
>Listen to her (she explains why she came to the room, her hidden jealously towards Lilly, and her tragic back story. Hanako breaks down and cries and ask Hisao to fuck her. Obviously a bad end.)
>She's drunk. Get her off of you and force her to leave. (Hisao manage to get Hanako off of her and kicks her out of the room. Leads to either the good end or netural end depending on previous choices.)

And to further drive the BAD END with the COMFORT DRUNKEN HANAKO choice home, the next day the police shows up and arrest you for rape (taking advantage of a clearly drunk girl = rape, you know)
