Sure, you could get fancy and rig it up with a fleshlight or something, but that's not the intended purpose. It's a body pillow, you just sleep while hugging it.

For regular dakimakura, yes (and these do appear to be the regular kind). But there are versions that are, shall we say, modified to be used in the way HarvestmanMan describes.
