A noobish question, I suppose

Alright, I have been browsing the forum for the past week and have picked up on most the "inside jokes" abut the characters.
The one I don't get though is when even Hisao is mentioned someone says "too bad hes dead"
is this a reference to the Kenji end, or is there a comic somewhere I missed?

Wait, Hisao's dead?
But seriously, somehow I feel like 4chan would be to blame for that. I haven't really noticed it though.

Katawa Shoujo is actually a retelling of The Sixth Sense.

Lilly: "I don't see dead people."
So, who do you think is actually a ghost?
(As for the actual question: it's probably a reference to the "Bad End" scene of Act 1, which happens if you don't manage to get close to one of the girls by the end of the first week. There's a picnic on the roof with Kenji, at the end of which you fall to your death)

He's not dead, he's sleeping.

But with strange aeons, even death may die.
Wait, is Hisao a Great Old One?

That sure doesn't refer to Lilly's zombified looks at all.

Thats just one example IO remeber, I've seen similar comments on multiple pictures of Hisao

Ehh? No, I was thinking more along the lines of the "Dead Parrot" skit from MP.
“He's not pining, he's passed on. This parrot is no more. He has ceased to be. He's expired and gone to meet his maker. He's a stiff, bereft of life, he rests in peace. If you hadn't have nailed him to the perch he'd be pushing up the daisies. He's rung down the curtain and joined the choir invisible. This is an ex-parrot!”
Old school comedy and all, but so am I.

Quite simple.
On the shimmie, there are multiple counts of Hisao coming into contact with... altered versions of the girls (Look under the nightmare_fuel and yandere tag). As bland as our cripple saviour may be, chances are he is either going to be devoured, stabbed, poisoned, shot, anally raped, burned, frozen, blinded or otherwise prevented from passing GO on the monopoly board of life.
It's not really a meme, as such. Just applied sense as to how a heart patient would survive a zombie apocalypse or a psychopathic girlfriend.
IE: Not.

"Too bad he's dead is a 4chan /co/ meme. It comes from a comic where Iron man is reflecting on Captain America. I think the quote is "I guess Captain America was right. Too bad he's dead."
