I'm not too sure about Rin's ending. I would have wanted to discourage her from working with Nomiya, cause I could clearly see he was trying to force his dreams upon her, at least, that's how I would have saw it. The storyline would have made a complete turn from "I would like to join the art club" and end up at some other place. I think I would end up on a good ending though. I would have figured out how to deal with someone like her.

If I was put into Hisao's situation as I am now, first off, I'd take the Arrhythmia in stride. Between my own tetralogy of fallot, craniosynostosis, and several other relatively minor medical problems over my lifetime (have since been repaired), a medical condition that could actually kill me wouldn't change me that much. To be entirely honest, I'm already living on borrowed time as it is and if they hadn't been repaired, I'd be near the end of my rope. That being said, I tend to be pretty laid back. Some of my friends even say that I have a calming atmosphere about me. I've mentioned before that I was bullied throughout middle/high school so I could somewhat relate to Hanako. I'm not much of a runner but I love to ride my bike through town and I often pass cars on my daily rides so I could see myself hanging out with Emi. If I ever find something worth the effort, I go completely gung-ho about it and tend to introduce people to it in the process (KS, Mass Effect, and a few anime come to mind) so I can also see myself spending time with Shizune. I like to help people. Just about a month ago, I helped a friend who tends to have panic attacks move into his new house and he said that I was one of the most calming people he has ever met even when faced with some drunk idiot almost plowing into us on the highway on our way to his new place. Calm and helpful seem to match Lilly fairly well. Rin however... I got nothing. My mind has been irreparably bent thanks to Japan but beyond that, nothing. I'm rambling more than I intended so moving on...
Even though I said that if I had a possibly lethal medical condition wouldn't bother me much, I would still be concerned enough to improve my chances so I likely would have taken up Emi's offer to run the track or at least hit the bicycle machine several times a week. I consider myself a techy so I could possibly appeal to Hanako's techy side as well. I have the energy to keep up with Shizune, no matter what she may be doing and I like to enjoy a calming peace when I get the chance in between whatever life may throw at me. Ultimately, I don't know who I would end up with but I am very confident in saying that I could stay on her good side.

If you read the VN you'd know that when Emi tried to hug Rin, she was kicked, and for some reason, Rin can kick really hard. If someone she wasn't close to tried to hug her, then ouch. If a GUY she's not close to tries to hug her, enjoy being infertile.

So we're assuming we're taking Hisao's exact spot, right?
I can tell you one thing for sure, if we were just playing a student in that school, I know I would have just kept my head down and gone through the entire class without making any connections. It's pretty much how I went through school, myself, so I can't see that changing there.
Now, if taking Hisao's place with Shizune and Misha making that first step to get to know you, things would likely have turned out different. I'd likely be on student council, but I doubt I could have learned sign language for Shizune, and as such if I did come to like her, I never would have said a thing. So... actually it might be a lot like the previous scenario, except having made some real friends.
Which, actually, is for the best when I think about it. Considering how often I put my foot in my mouth in NORMAL social situations, I can't even begin to imagine how badly I'd mess if I was in Hisao's shoes.
