I think about it this way: I'm not the one deciding to have sex, that's between Hisao and the girl(s) he's with. if anyone had gotten their disabled groove on in act 1, that would have felt really, really rushed - nothing's developed enough so far for any sex to be anything but gratuitous. the game is so very much about the characters, not titillating the player. (yeah, I'm using lots of big words, had an English class today.)
so if it gets to the point when it feels right for the characters to get intimate, and they think they're ready for it, then it's all good. there was some speculation on another thread about when in the later acts sex will happen, and people were figuring based on the girls' personalities whose path would have sex the earliest. lots of people thought either Emi or Rin would have it earliest, pretty much everyone thought Hanako would have it latest, that sort of thing.
besides, there's probably going to be the choice to have sex or not, at least in some of the paths - none of the girls seem like the type to drag you to their beds, shouting "Dammit, I have needs!" (Okay, maybe Shizune, but it's not like she can shout and drag you at the same time.) and I think one of the side effects of Hisao's meds is erectile dysfunction (pills of boner-killing), so he'll probably have to go talk to Nurse if he wants to have sex with anyone. and if
that's not a scene begging for a "are you ready for this, Hisao?" talk, I don't know what is.

Does anyone know (or will the devs tell us) if the game continues to play out every single day? That is, for a three month relationship, you'd play 90 or so days (without a "one week later..." title or something to skip time). If so, I think it will
feel like a long enough time has passed to wait for sex, at least from the player's point of view...

That would be awesome, but that seems like it would be a ton of work, and I don't think there would be enough material to have 90 days in the life of Hisao. It would probably seem a little long as a player.
There will probably be timeskips. How those are handled will most likely differ based on what the writer wants, so each path may have different time skips. Or maybe the writing team (Aura) decided to have it be like act 1, where some time gets skipped after act 1, and then we play another week straight in every path.

hmm.... after the first I kinda tough that stuff that make Hisao's heart race (like confessions, intimacy etc.) might cause a heart attack or at least give a crazily higher possibility for an attack.
Ergo it's possible that sex might just kill him, Nurse's effort going to waste ta daa. bad end constructed.
Would be a really really sucky disease if that's the issue...

I remember one of the devs saying that Hisao is going to have to take it slow at first, because of his heart. He won't be 'fucking like a stallion' any time soon.

After Act 1, the paths utilise timeskips between scenes. There's no outright statement of how much time has passed before the scene starts, just a general clue within the scene itself as to when it takes place.
The original page's timeframe was more or less a tether to the timeframe actually used in-game. Each path covers a different length of time, some spanning a longer timeframe than others, but none vary overly significantly from the three month limit from the original page.

I've never had a problem. I'm on many of the same types of drugs Hisao is on, for many of the same reasons (based on the scene where his symptoms scroll past). If he can jog around the track he can have non-vigorous sex. As G3n0c1de said, if he takes it slow he'll be fine. Let the girl be on top, that will take away a lot of the stress. He also has weeks or months to exercise and build up stamina before it becomes an issue. Having a heart attack after pushing yourself on day 2 or 3 of an exercise regime doesn't mean moderate activity on day 40 will be a problem.
As for the medication side effects causing limpness...call the doctor and change the prescription. He's very early in the cycle of treatment. They're still looking for what works for him. If he told the doctor he was having bothersome side effects, any bothersome side effects, the doctor would change the medicine. If he refuses, call another doctor because that one is too arrogant to be helpful. There are dozens of different medicines that give the same end result so until he has tried them all, the doctor would change the prescription until they found one that worked for him. Even then, even after they have tried every beta blocker and antidepressant available (the two types, of those listed, most likely to cause sexual side effects) and he had a bad reaction to every single one, there are other types of drugs that use a different method of action entirely. He is years away from needing to make the choice to live with a lifestyle altering side effect or to live without medicine. It is common to have some sort of side effect from new medicines, it's uncommon to be able to do nothing about it. Though in the short term it's a funny scene. I can definitely see Hanako getting very self-conscious about it happening with her. And that thought makes me giggle, as does the scene where Shizune gets very frustrated with Hisao for his failure and then uses her hands for something else...

Nooo, Viagra/Levitra and similar drugs can really help (I'm on some heavy duty high blood pressure drugs). There's a reason they sell for $10 a pill.
Aside from that, Hisao also just hates how his condition keeps making him look weak in front of girls. Just like Hanako (and to a lesser extent Emi and Rin) are self-conscious about their physical appearance, he's self-conscious about his.

but seriously, great points there, Wokka. it's been pretty clear that the devs have done their research, so I'm guessing that whatever happens, it won't be all wildly unrealistic or the kind of thing that makes you go "Wait, what happened to that whole heart thing?" I don't think he'll get into a major medication discussion with Nurse, at least not one that we see ("We talked about medications and their effects for an hour. About twenty minutes in, I stopped being embarrassed about why I wanted to get on new meds."), but I'm sure it'll come up.
also, the thought of Shizune getting all impatient is hilarious. I think she'd be understanding, kinda, but her first reaction of "You can't what?!" would make me laugh my ass off.

That might be an interesting scene actually. Being in hospital and then on dosages of medication really hit him hard; and I think it's safe to assume that it'll be his first time as well, so he'll actually be more nervous than many other guys would be. In some cases he'll likely be more apprehensive than the girl he is romancing, for any number of reasons.
On that note, I can imagine part of at least one or two H scenes for certain paths being the girl of choice studying the scar on Hisao's chest from the operations in some way or form.
