I can't see, so I'm going to assume.

So i’m an idiot, I had to stare at this picture for like 45 seconds before I could work out why it looked like Lilly’s legs were facing backwards.

It's pretty obvious that both of Hisao's hands are busy holding Hanako, and Lilly's got a hand positioned to take care of herself.
