My version of Alt-Rin probably would have been a very quiet girl. Not shy, just quiet and introverted. (Basically going off the description of her as "The most difficult one to get.") I probably would have done a scene similar to the one in the atelier where Hisao feeds her oranges, but it would have been more of a proper meal. (And I probably would have gotten rid of the thalidomide thing, since otherwise this game would have to be set in the late 70's.)
I might actually still have made Alt-Lilly and Alt-Hanako friends, because of the whole "girl who's self-conscious about her looks and girl who can't see said looks" thing. And if I was going to make one of the girls a cloudcuckoolander, it probably would have been Alt-Lilly. She just looks a little spacey in that image. To me, at least.
As for Alt-Emi, the accident probably wouldn't have been a fatal one. I might even say that she'd been hit by a car while walking, rather than while riding in another car. I also might have made her and Shizune friends, since they both come across as go-getters in the original descriptions.
I probably would have allowed the player to attempt "gigolo play," but like the description says, it would end badly.
Assuming I gave the protagonist a medical condition, it might or might not have been a heart condition. I might have gone with hemophilia, severe asthma, or something else that, like a heart condition, is potentially life-threatening but relatively invisible.
Side note: I would probably have a character with cystic fibrosis, as a nod to a friend of mine with that condition.
