Simple: She didn't. It's physically impossible to be underneath and above someone simultaneously. Also, being on top and being a top are two different things.

And I was referring exclusively to same-sex relationships with the "Protagonist is always top".
In almost all VNs with Yuri/Yaoi content, the Protagonist is the dominant one in the relationship (Top/Seme)

Seme- the dominant partner in a same-sex relationship (gives)
Uke- partner on bottom, submissive one in relationship, usually younger and less experienced the the Seme (receives)

Examples of a Seme:
Kanou from Okane Ga Nai
Usui from Maid-Sama! is a heterosexual example
Shizuma of Strawberry Panic!, is a yuri version of the seme archetype.

Examples of an Uke:
Sawada Tsunayoshi from Katekyo Hitman Reborn!
Yumi in Maria-sama Ga Miteru
Momo from Kimi Wa Petto is an example of the almost unheard of heterosexual uke.

No, no. See, protagonists always top. Ergo, Hisao simply allowed her to pretend to top, because he felt like not being in charge for a while.

Also, if she was on top, she couldn't have done anything from underneath him.
