I don't wanna burst your bubble, but years of watching CSI and FBI Files tells me that if he used chloroform and raped Hisao before tossing him off the roof, it's unlikely they'd just see it as an accident. So the flight trajectory from the airport head towards Yamaku ... the ensuing BOOM cashed by a few tons of metal smashing into the ground ... a crash hard enough to kill those onboard the flying metal coffin ... is close enough that Hisao in the dorm can get to it ("Oh hey, my girlfriend left me, I have nothing to do but investigate that loud semi-earthquake I just heard") ... before any police/ambulance can be there.

Shizune Neutral: After comforting Misha, Hisao comes clean and confesses to Shizune. She is greatly hurt, but they are able to maintain at least a courteous friendship
Lily bad: Hisao succeeds in catching Lily at the airport, and begs her to stay. She cries that she can't, and yells at Hisao for putting her in this position. As Hisao watches her walking away, the heart pound animations flashes twice. <end credits.>
Shizune Neutral: After comforting Misha, Hisao comes clean and confesses to Shizune. She is greatly hurt, but they are able to maintain at least a courteous friendship

Lilly ending: After Lilly leaves. Hisao goes into full hikkomori mode and refuses to leave his room.
Hanako comes by everyday trying to talk with him, after a couple of days Hisao lets her in.
Hanako does her best to try to cheer him up, after she finally manages to get a smile out of him, her eyes get teary.
A worried Hisao asks what's wrong, and Hanako tells him that she has been jealous of Lilly this whole time and that she's in love with him.

On topic: I was honestly cool with the lack of a neutral or bad ending for Shizune and Lilly, respectively. However, I'm down with an EXTRA-GOOD ending for Shizune where she realizes that Misha's kinda hawt and Hisao sucks at this game.

Shizune Neutral: As Good End, but less of a positive spin. The three friends vow to stay close, but Hisao has his doubts about it, noting that it is unlikely the three of them, going their own paths of life, will ever see each other again.
Shizune Good: Shizune and Hisao decide to go to the same college. Misha still leaves, but vows to keep in touch, perhaps with an epilogue of her visiting them at school a year later.
Since we're adding a new ending, there would be new choices in the route.
One choice that locks you into a Bad End was the greatest failing of Shizune's route
Lilly Bad: As written, Hisao alone in the hospital
Lilly Neutral: Hisao and Lilly stay in contact and try a long distance relationship, but it eventually falls apart. Told with a positive spin, they stay friends and don't regret the time they had together. Hints at the end of Hisao and Hanako growing close.
Lilly's route was written as painfully sugar sweet. Even the Neutral End gets a happy ending, if not a stereotypical one.

Shizune's 3rd ending:
Shizune finds out (or heavily suspects) that Hisao "comforted" Misha. Shizune is terribly hurt, but her analytical mind kicks in and works out: 1) she can lose them both by rejecting them both, or 2) she can keep them both by accepting them both. She offers this choice to Hisao and Misha, if they want to be part of a triad relationship. Misha eagerly accepts, as she still loves Shizune and she is fond enough of Hisao to make it work. Hisao's choice determines the good or bad ending (the "Don't Comfort Misha" ending being the neutral ending). If he says yes, then the three stay together forever, get into the same college, and discuss the implications of how they can continue to be together in a society that will not openly embrace that.
Lilly's 3rd ending:
Hanako and Hisao discuss Lilly leaving (the second leaving). If they both accept Lilly leaving, then queue the bad ending. If Hisao fights it, then queue the good ending. If Hisao accepts it but encourages Hanako to fight it (figuring that Hanako may have more influence than himself), then Hanako is successful in keeping Lilly in Japan. This is the neutral ending, where the three of them must now form a new dynamic amongst themselves. The implication is that the three have equalized their relationship with each other, and it leaves it up to the reader (with several different hints) on how the three continue together (with both platonic and sexual aspects up for grabs).
