The Nurse would be female, obviously. Somehow I can see her being called "The Doctor" or something, instead. There's this famous experiment that found out once that most people think of a man when they think of a "doctor". This way f!Hisao can still be surprised upon seeing her due to expecting a person of the opposite gender.

Eeh, no. I'm a straight female and I still wouldn't play that. I don't like bishounen genres at all. They get really cliche, really fast (in my experience.) But I'm always willing to be proven wrong.

If Lily was a guy the character would change from a royal type woman to a rugged sort of guy with defined facial features always holding a coffee mug and staring into the distance, with an empty stare and a poor disposition. And he would have a five o' clock shadow, aaaaaaaand an aviator jacket.

This. Just this. Extra points for the "empty stare off into the distance" because this would be an excellent set up for when female Hisao first meets male Lilly (remember, near the beginning of Act 1, Hisao wasn't immediately aware that Lilly is blind.)

Also, I'd think male Misha would have a fair amount of stubble on his chin, as well as a drill ponytail hairstyle.
