Yep, it's a flag in all cases. I tested it on the Shizune route - gave responses that would all lead in her direction, but chose "talk to Hanako" instead of waiting for Shizune and Misha to finish bickering. I ended up getting Emi's ending.

That should be "Clouds in my Head", Rin's Act 1 Ending.
Sometimes the game seems to glitch, not activating scenes you've already seen. Just playing the scene again usually fixes that.

The last picture? That's the 100% completion CG.
You get it for unlocking every scene, plus the 13 end credits.

I have 100% completion. I have done every choice possible. What did I do wrong?

The most common reason is people didn't get Lilly's bad ending.
I think it is also possible to unlock every scene in Emi's path without seeing her bad ending, so check if you got that.
