It might just be how she's been positioned in this frame - after all, there isn't actually a scene where Misha is in Shizune's bedroom with Hisao is Shizune's naked... Is there?! Although there does seem to be an awkward lean that'd make her fall backwards and to the right, I'll give you that.

That might just be the text box throwing your frame of vision off slightly, although her waist is particularly small.
