OT: I think it would have been interesting if Iwanako showed up in Shizune's story rather than the comforting misha choice. Hisao would have to choose between Iwanako and Shizune.

If Iwanako came over to Yamaku, it'd probably merely result in awkwardness. Most of the time when Hisao gets her letter, he doesn't really take it all that well. There's no reason to believe he'd take her physical presence any better. Not to mention she'd be awkward as well. Both Hisao and Iwanako kind of, more or less, somewhat moved on with their lives. The letter was meant to finalize that process. (at least to Iwanako)

Iwanako confessed her love to him. From her perspective she almost killed Hisao, even if she could not have anticipated that of course. As far as we can tell, she never got any kind of closure on either of those points. From the description of the hospital visits she only moved on quite a while after her efforts had become futile. I don't think it's Iwanako who did anything wrong there.

I agree with you wholeheartedly. Thinking you made some person suffer even if you did it unconsciously and didn't meant it, the guilt can gnaw you to the bones for your entire life. And this person being your beloved one makes it even sadder.
Honestly, if Iwanako transferred, it'd all just get really awkward. The REAL question though is: What would she look like? We never see a canon face...
