What hurts the most? I think there are a lot of things that hurt, all in generally equal value. If I had to pick one it would have to be seeing Rin, feeling the wave of emotions flow over my heart, that rush you get in your chest, then realizing that I can't touch her. That she isn't real. That hurts a lot. But i'm glad that I feel something, even if it's just for brief moment.
I have an involuntary reaction to comfort. I will cry no matter what. But it doesn't really hurt.
Inversely what feels the best is ironically seeing Rin. Which is then followed by the the thing that hurts the most. Seeing Rin.

Well, that's certainly a great way to see it :D
I feel that whatever Rin would say would create a different response from Hisao.
I'd imagine it going something like this.

Rin: Hisao...can you come over?
Hisao: Sure, just let me get dressed. What's the deal?
Rin: ...
Hisao: Rin?
Rin: I... uhh...
Hisao: ...
Rin: I'm horny, okay? Now get your but over here and please your woman!!
Hisao:

Yeah, I'm not sure if that's exactly how a phone call with Rin would go, since she would probably have Emi hold the phone to her head :P
But I'd imagine Rin saying something like that...maybe?

There's not much in the game that I found hurtful. I love drama and angst in stories, so the scenes with the most intense sadness and anger are often my favorites.

The death of Emi's father was painful, though, since it made me think about if I lost my dad in such a way. Shizune's entire route after comforting Misha was such an avalanche of misery that I did begin feeling their stress eventually (though I still love it for being perhaps the most intense stretch of Katawa Shoujo). Rin's route was depressing in that Hisao's relationship with her became so strained that at times it seemed irrepairable. Certain aspects of Lilly's route, like the parental abandonment and Akira breaking up with her boyfriend, bummed me out worse than most other sad elements of the story because they were hansled in a subtle, low-key fashion and there was no strong emotion to feast on. Emi's Bad Ending was cold in its finality.

You might want to see a doctor.

Ignore what I said earlier. Shizune's bad end hurts the most. That, and Emi's ass after the shed scene.

Actually what hurts me the most is imagining what sort of Hell one little girl who just lost her father and her legs, and another who suffered through the agony of burn recovery, only to find out her parents were dead must have gone through.

Imagination is a bitch sometimes. :(

Emi's very attached to her father. And Hanako needs to embark on a family tree research.

She was 8 when it happened, so she would know any immediate family members.
I am the male version of Hanako Ikezawa.

I actually had a fanfic idea of Hanako's distant relatives: Her grandpa (on which side is up to you) had a brother. But, the brother became a yakuza. Given the stigma, Hanako's grandpa broke off all relations. The brother decided on his deathbed to entrust his son (and grandson) to find out more about the family tree. Basically, it's a "Hanako becomes a yakuza princess" story. A bit of hijinks, but can be milked for drama for all its worth.

I mean not when she's 8. But, now as she is older, she might be a bit more curious about the extended family. Granted, it's at least her grandparents' or great-gramps' side of the family. But, at least there is a link.
