So I have been playing Katawa Shoujo this past month and like 2 days ago I completed all the arcs and was proud because I thought I got 100% in the game.
But for some reason I am only at 99% even though for all the girls I have all their chapters. And I already got the bad ending with Kenji.
The empty chapter in the full library comes after 'Home field advantage"
I am wondering is there possibly a 'good ending' with Kenji, because I am lost

The OP is talking about the library and not the gallery that's missing an item.

If I'm correct, that particular item is triggered by going all out during the second run with Emi but you need to have triggered enough flags with Shizune to get that scene.

Absolutely correct, demonix.

The scene is called 'Slow Recovery'. In 'Lunch Evolution Theory', choose 'Wait for Shizune and Misha…'. In 'Waylay', choose 'Yes'. In 'Cold War', choose 'Don't drag me…'. Finally, in 'Exercise', choose 'Go for it'.
