I think you're reading into it a bit much. I think she burst in and called the nurse an asshole because she knew his joke meant that they were done. "Asshole" almost seems to be Emi's term of endearment for the nurse.

For me, "explain" was the non-committal one, the one was letting Rin free to do what she could, and the onus to understand was on Hisao, while "I need to understand" sounded more like an ultimatum, putting pressure on Rin I didn't want to.
